You may file a commendation for a Department employee by any of the following:

1. You may contact Internal Affairs Group (IAG) by phone at 1-800-339-6868, or you may visit the IAG office downtown in the Bradbury Building at 304 S. Broadway Street, Room 280 Los Angeles, CA 90013.

2. You may complete a Employee Commendation form, available at any Los Angeles Police Department Community Police Station, the IAG office, the Office of Inspector General, any City Council field office, or at the LAPD website http://www.lapdonline.org/home/content_basic_view/37655.

3. You may visit any LAPD Community Police Station and speak directly with an LAPD supervisor, regardless of where the incident occurred.

For your convinience we have provided a PDF* fill-in Employee Commendation form that allows you to type your answers using your computer keyboard which you can then print and then mail or take it in person to any of the above addresses.
